Changing your details
If you need to change personal details such as your address or contact number, the easiest way to do this is through the NHS App. This is a free app which you can download from your phone’s app store (Android and Apple devices).
If you have not logged in before, you will be asked to create a new account, which you will need your NHS number for. You can find your NHS number here: Find your NHS number - NHS (www.nhs.uk).
Alternatively, you can speak to the reception team in-person or by calling the practice. Try calling after 11am to avoid busy times on the phoneline.
Sharing your medical record
To help improve the treatment you recieve, we may share your medical data with other healthcare services. This allows us and other providers to have a better understanding of your health history in order to provide better care.
All data that is collected and shared is protected by strict rules around privacy, confidentiality and security. Your data will never be sold to insurance or marketing companies.
If you would like to opt-out of data sharing, you can download a Type 1 Opt-out form and return to the practice. Please be aware that opting-out will prevent you from using certain services, such as GP+.
